Individual Request for Review by AGO’s Justice Review Unit

This form is for an individual prosecuted in a criminal case by the Attorney General’s Office in Massachusetts. You must either have been found guilty after trial, pleaded guilty to an offense, or had a guilty plea imposed after initially receiving a continuation without a finding (CWOF).

To request a review, you must now have a claim of:

          a. Actual innocence (no role in the crime);

          b. Actual innocence (new evidence to prove innocence); or

          c. Legal innocence (actions justified through self-defense, defense of others, or mental illness; culpable of lesser crime; senior trial error or discovery violations; or illegal or unjust sentences).

Upon receipt of your completed application, the JRU may request that you authorize the attorney who represented you in the criminal case under review to discuss the events that led to the conviction. The JRU cannot contact your attorney unless you give written permission or sign a waiver provided by the AGO. The JRU cannot require that your attorney speak about your case without your permission. We may also disclose your request for review and related information to other law enforcement and parties involved during our investigation.
