Acknowledgement

Please read the Tool Barn Guidelines prior to completing this request form. Equipment may be signed out for an initial duration of two weeks. Extensions may be requested by contacting the Tool Barn at toolbarn.dcamm@mass.gov and providing the status of the project, reason for extended use, and the expected date of return. Individual forms must be completed for each piece of equipment requested. If equipment is damaged or inoperable for any reason, participant will notify DCAMM’s Central Region Office immediately by telephone (978-365-7352) and by e-mail (toolbarn.dcamm@mass.gov) and will submit a “Tool Barn” accident report and any police reports, if applicable, to DCAMM’s Central Region Office via email or fax (978-368-1511) within 48 hours of the incident.
