Instructions: Fill out the form below (*indicates required field) to request registration of your apiary and honey bee colonies with the Commonwealth. A copy of the completed form will be automatically emailed after online submission to the email address provided. 

To remove your apiary from the registration system, you must submit an additional form and select the “Remove apiary” option for each address you previously registered.
