The MassTrails Program requires that all equipment purchased with grant funds be reported on an annual basis for a period of five years following the grant contract end date, as indicated in the MassTrails Grant Agreement document. Please note the date of this report and submit subsequent annual equipment reports for no less than five years going forward. Please use this form when submitting your reports. If you fail to report on the equipment as required, or the status, condition, or use of the equipment is considered out of compliance at any time, MassTrails reserves the right to re-possess any and all components which were purchased using state or federal funds.
