MassEVIP Multi-Unit Dwelling and Educational Campus (MUDC) Charging Program Application

Eligibility

Please answer the following questions. All documentation and records submitted in support of funded applications shall be made publicly available to the fullest extent provided by law.
Is the project location a multi-unit dwelling or an educational campus?(Required)
Has the applicant already ordered and/or purchased the charging station?(Required)
If yes, you are NOT eligible for funding through the MUDC Charging Program.
Are there 5 or more residential units located at the proposed project location?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
Does the applicant agree to allow practical access to, and use of, the EV charging station by all residents onsite?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
Are there at least 15 students on site?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
Does the applicant agree to allow practical access to, and use of, the EV charging station by all students and staff onsite?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
For each port installed, will the applicant designate at-least one parking space for plug-in electric vehicle use only?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
Is the proposed charging station able to charge electric vehicles (EVs) produced by multiple EV manufacturers?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.
Does the applicant own the location identified in this application where the charging station is to be installed?(Required)
If no, does the applicant have evidence that the charging station installation is allowed on the property e.g., written permission of owner or pertinent language in lease, license agreement, or easement, etc.?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program.

Is your Electric Vehicle Supply Equipment (EVSE) listed in the State Appliance Standards Database at (see links below) OR is it a DC charging station of 36kW or less?(Required)
If no, you are NOT eligible for funding through the MUDC Charging Program. For more details see links below.

W-9 Form

The applicant is required to upload a copy of a current W-9. Applicants may submit either the Federal IRS March 2024 W-9 available at link below or the Massachusetts W-9 containing the footer “Massachusetts Substitute Form W-9 (Rev 4-2022)” available at link below. Older versions of the W-9 will not be accepted. Your application cannot be processed if you submit an older version of the W-9. The W-9 must be signed and dated within the last year.

There are three types of electronic signatures that will be accepted on the W-9: 1) A scan of a traditional “wet signature” (ink on paper); 2) Electronic signature that is either: hand drawn using a mouse or finger if working from a touch screen device; or an uploaded picture of the signatory’s hand drawn signature; 3) Electronic signature affixed using a digital tool such as Adobe Sign or DocuSign. Typed text of a name not generated by a digital tool, computer generated cursive, or an electronic symbol are not acceptable forms of electronic signature.

    In the attachment section please provide submitted application or, if utility program application has been approved, attach approval letter or equivalent.

    Latitude and longitude

    Provide the latitude and longitude (x/y coordinates) for the project location. If you’re not sure how to get the coordinates they can be found at: Google Maps - instructions: Open the link below on your pc or mobile device Enter your facility address or location Right click (pc) or tap (mobile) the specific location of the planned project - a popup window should open to display the coordinates. When you click on the coordinates in the popup window it should automatically be copied to your clipboard. If using the google maps app on your mobile device, touch and hold the specific location of the planned project and a pin will be dropped. The coordinates will be in the dropped pin information window, touch them to copy to clipboard.

    Be sure your coordinates are in decimal format, with at least 6 decimal places. There should be a “-” at the front of your longitudinal (Y) coordinate. A local coordinate for example, would be (X) 42.275404, (Y) -71.303471, not 71303471.000000 or anything else.
