Reporting Form

Instructions: Please fill out the required fields below, including a description of the project or projects that your municipality either has completed or intends to complete using the disbursed funds. In this description, include the amount of funds your municipality has spent or proposes to spend on each element of the project. If the project or projects have not been completed, please state when you expect it/them to be completed. If your municipality has not determined a use for the funds and wishes to bank them for future use according to the statute, please indicate so in the description box. Please note that your municipality is not relieved of its obligation to report the spending or proposed spending of the funds once a plan has been developed.
