On this form, authorized individuals are able to request new highway grade crossings or modifications to existing grade crossings in Massachusetts from the Massachusetts Department of Public Utilities (DPU). Authorized individuals may also submit general inquiries. Please note the following before proceeding:

This form can be used to request new highway grade crossings or modifications for existing grade crossings in Massachusetts, not in other states, territories, districts, or countries.
This form can also be used to submit general inquiries about grade crossings in Massachusetts, but not grade crossings in other states, territories, districts, or countries.
This form cannot be used to request new private crossings or modifications of private crossings.
This form cannot be used for safety complaints or concerns by members of the public.

This form can only be filled out by authorized individuals.
- Some examples of authorized individuals are those employed or otherwise authorized by:
  - the Massachusetts Department of Transportation (MassDOT)
  - a Massachusetts city or town, including municipal consultants
  - a County Commission
  - the railroad carrier owning or operating that section of rail
This form asks for information about the new crossing or modification request, along with attachments.
This form may ask for an authorization and concurrence. The 'Save & Continue' feature may be used to save your progress and send this form to another person to receive authorization and/or concurrence.
- DPU reserves the right to request a wet signature or otherwise verify identities before further processing your request.
