Thank you for taking this step to confirm the eligibility of a location for a mixed-use development district in connection with Section 3A. Please note that a positive determination for district location is limited in scope to the district’s location and does not constitute approval of a district’s compliance with the criteria set forth in Section 5.c. of the Regulations. This form must be submitted by a municipal official in order for the location to be evaluated. If there are multiple locations that need determination, please submit this form for each location.
