In order to restore access to your UI account we are going to have to verify your account and identity. You will need to provide documentation to verify your identity and provide proof or your address. Please complete the form and upload the requested documentation.

Please upload clear, color copies of the following: 

  • Front/back of State Driver’s license or State ID, Passport picture page, or Permanent Resident Card – State Driver’s/State ID is preferred. 
  • Front and back of Social Security card 
  • Proof of current address – this should not be a P.O. box 

Additionally, in order to expedite the verification process the person can provide: 

  • a selfie with the ID card 
  • Best phone number to be reached at, if additional information/documentation is needed.
